Start by choosing which Rx Program you want to enroll in, and select "Enroll Now".
Find your state from the dropdown to view available plans.

Select one of the available plans and click "Next".
Continue filling out the application with the required information, including applicant details, address, payment info, and effective date. Review and submit to complete.

Upon completion of the enrollment verification process, you will receive a Welcome Email that includes your ID cards and important links to access your member account, plan benefit details, and drug lookup and pharmacy locator tools.

The formularies provide coverage for select generic and brand name drugs in every medically necessary therapeutic drug class, excluding specialty drugs. Not all generic and brand name drugs are covered under these formularies. Please ask for a copy of the formulary to see if your drug is covered. Service in nearly all 50 states.
